Members Form

The purpose of this form is to gather information about you on behalf of all Dead on Arrival club members. By filling out this form you are requesting an invitation into the Dead On Arrival Warhammer Club. You MUST be 18 years or older to join the DOA Warhammer Club.

After you have filled out the members form you can expect an email within a couple of days from one of our club organizers and a follow up phone call.  You should jump over to the Club Forum and register a user account and introduce yourself to the other club members in the Visitors Forum while you wait for your membership approval.  Once approved your forum ID will be updated to Club Member status and you will gain access to post in other areas of the forum. This member form will be added to a list and presented in a secured area of our site to the rest of the members.  You will also receive a members card in the mail within a week or so of approval.

Please note the club launch date is April 4th, 2010.

Dead on Arrival being a social club expects its members to behave according to a certain code of conduct whenever interacting with DOA staff or fellow members wether its online or imperson. We refer to this conduct as "the honor system". In short we ask that all members are always honest, polite and fair when dealing with the club organizers and fellow club members.
